We might have thought that this was a design failure, but on a closer look, it appears that it was a maintenance failure. The empty bolt holes suggest that the final piece of the slide is missing. Non-impact absorbing surfaces, such as concrete, asphalt, grass, gravel, or soul, are one of the things to stay away from when on a playground.
Slides like this are supposed to be safe for kids to play with, and one of the mechanisms put in place for this is having a soft landing. Some surface materials that are shock absorbing include rubber mats, wood fiber, artificial turf, and synthetic grass—these should have been used instead!
